Community Nursing program for veterans
The Community Nursing program is designed to promote independence and to improve the health of veterans wanting to stay living at home as long as possible through the provision of quality nursing and healthcare services. It is different to the Veterans’ Home Care program as it aims to assist those more with clinical needs. It can be used in conjunction with other veterans’ support programs.
It also aims to ensure veterans have their specific health needs (for example injuries received in defence force duty) and concerns met in their home, reducing the likelihood of needing residential care or hospitalisation down the track.
HomeCare Options is funded by the Department of Veterans’ Affairs (DVA) to provide veterans with nursing and in-home support services through the DVA Community Nursing program.
Community Nursing services
HomeCare Options Community Nursing offers a range of clinical and in-home support services. Eligible veterans are able to access services such as:
- medication management
- case management by HomeCare Options nurse including regular review
- personal care such as help with personal hygiene or dressing
- rehabilitation services
- continence management
- management of chronic diseases
- wound care
- palliative care
- pain management
- help with accessing support for carers or referrals to other services in the community
Community nursing services are delivered by HomeCare Options registered nurses, enrolled nurses and nursing support staff.
How do I access this program?
You will need a referral to access services under the Community Nursing program.
To arrange for this referral you can call the Veterans’ Home Care Assessment Agency directly on 1300 550 450. You can also ask your GP for a referral. If you are in hospital a discharge planner might also refer you to this program. When you arrange for your referral, you can ask to be referred to HomeCare Options as your preferred provider.
If you are already receiving services from HomeCare Options, you may be eligible to access services under this program. If you are an eligible veteran and feel this program may assist, please speak to your case manager.
To be eligible for the DVA Community Nursing program you must be a veteran of the Australian Defence force, hold a Gold or White care and be living at home. The VHC assessment team will determine your eligibility.
